First randomized clinical trial shows efficacy of intravenous ketamine for treating Posttraumatic Stress Disorder
A team of researchers put intravenous (IV) ketamine therapy to the test in forty-one patients with chronic Posttraumatic Stress Disorder (PTSD). Their results were published in the Journal of the American Medical Association Psychiatry (JAMA Psychiatry). This was the first randomized clinical study of its kind, pinning IV ketamine against an active placebo, IV midazolam. The primary outcome measure of the study was change in PTSD symptom severity. To this end, ketamine was associated with “significant and rapid reduction in PTSD symptom severity, compared with midazolam”. Interestingly, ketamine was also associated with a reduction in comorbid depressive symptoms and an improvement in overall clinical presentation, a novel finding in these difficult to treat patients.
Further, the authors found intravenous ketamine in this cohort of patients to be well tolerated without clinically significant persistent dissociative symptoms. This study provides the first evidence for the rapid treatment of PTSD using intravenous ketamine—a finding that will hopefully support novel approaches to the pharmacologic treatment of patients with this disabling condition.

Neuropathic pain is often very difficult to treat with current Food and Drug Administration (FDA)-approved analgesics. The most commonly prescribed medication to treat these chronic pain conditions is opioids. Opioids have very limited efficacy in patients with chronic neuropathic pain conditions. This has sparked research into new targets for the treatment of these patients. Promising research by investigators at the Cleveland Clinic demonstrated that microglial inflammation is a common pathway for neuropathic pain and other treatment-resistant neuroinflammatory conditions, such as Alzheimer’s disease (AD).
“Microglial inflammation is a mechanism of many CNS disorder – neuropathic pain, Alzheimer’s disease, multiple sclerosis, parkinsonism, you name it,” said lead researcher Mohamad Naguid, MD, of Cleveland Clinic’s Anesthesiology Institute, Department of Pain Management.
Subsequently, this team of researchers has synthesized a molecule called MDA7, which is a cannabinoid type 2 (CB2) receptor-selective agonist. Preliminary studies with this new drug candidate have shown that it can inhibit microglial activation and recruitment, which is fundamental to microglial inflammation and neuropathic pain.
Thus far, their research is in the preclinical phase. They have shown efficacy in a rodent model of Chemotherapy-induced neuropathic pain1, CRPS (not yet published), and some promising results in an animal model of Alzheimer’s disease2. They hope to test MDA7 in humans as a next step. The team is very hopeful that their preclinical results translate well to the clinic. Only time will tell.

Many Americans live with chronic pain in one form or another. About 52.5 million Americans report having been diagnosed with some illness that causes chronic pain, like arthritis, rheumatoid arthritis, lupus, gout, or fibromyalgia. The symptoms of chronic pain can be very severe and can dramatically affect a person’s quality of life. Finding relief of pain due to chronic pain syndromes can be tough, but according to Harvard Medical School, tai chi could help.
Tai chi is a martial art that began as a form of defensive techniques but has evolved into a method of relaxation and active meditation. Movements are slow and fluid, and people participating in tai chi breathe naturally and deeply while focusing on an area of the body just below the navel, which is said, in the practice, to store the body’s “chi.”
According to a study published in the New England Journal of Medicine, tai chi can actually help some chronic pain patients. There are a number of different chronic pain medications and procedures like nerve blocks, minimally invasive surgeries, and the use of ketamine to treat pain, but tai chi could be a good supplement to any of these, not to mention a way to keep active through the symptoms of chronic pain. Compared to a control group, a group of fibromyalgia patients reported less pain, depression, and more sleep after six weeks of tai chi.
In a survey that asked participants about the four most common types of pain, respondents said that back pain was the most common (27%), followed by severe headache and neck pain — both at (15%), and facial pain (4%). Americans spend about $86 billion each year on back pain alone, so finding effective methods is helpful for those who haven’t found relief any other way.
It can be hard, but staying active through chronic pain can actually help relieve it, and tai chi is an easy and low-impact way of doing it. It’s usually practiced in classes where participants learn the moves and exercises, and then do them alone at home.

Nocturnal leg cramps are sudden contractions of the lower leg and foot muscles. They often awaken you from sleep. The calf muscles are most often involved. The cramps are harmless. They do not mean that you have a serious disease.
The specific cause of nocturnal leg cramps is unknown. These cramps may be related to imbalances in local muscle chemistry. Many activities and diseases are associated with nocturnal leg cramps:
- Overexertion of the muscles
- Standing on hard surfaces
- Prolonged sitting
- Certain leg positions while inactive
- Dehydration
- Diseases
- Diabetes
- Parkinson’s disease
- Hypoglycemia
- Hormone disorders (eg, thyroid imbalance )
- Chemical imbalances (eg, calcium, potassium, magnesium)
- Certain medications
- Blood pressure medicines
- Statins (that lower cholesterol)
- Morphine
- Lithium
The following factors increase your chance of developing nocturnal leg cramps:
- Age over 50
- Overexertion
- Pregnancy
- Staying in certain positions for a long time
- Flat feet or other foot or ankle deformities
- Alcoholism
- Neurologic diseases
- Certain medicines
- Diabetes
- Parkinson’s disease
- Hypoglycemia
- Hormone disorders (eg, thyroid imbalance )
- Chemical imbalances (eg, calcium, potassium, magnesium)
- Sudden nighttime calf (or foot) cramps
Your doctor will ask about your symptoms and medical history. A physical exam will be done.
Tests may include the following:
- Blood tests
- Checking your foot pulses for good circulation
Talk with your doctor about the best plan for you. If no specific cause can be found, treatment options include the following:
- When cramps occur, pull against them with your leg muscles. Also, grab your foot (feet) and pull up.
- Standing on the affected leg often stops the cramping.
- Massage and hot or cold treatments to help the muscles relax.
None of these medicines has earned full approval for either safety or efficacy. Quinine , while often effective for reducing the frequency and intensity of cramps, has a significant risk of major allergic reactions. The other prescription medications carry risks as well. So, they are not generally recommended. They are most often used in only severe cases.
- Diphenhydramine hydrochloride (Benadryl)
- Calcium
- Quinine sulfate
- Simple muscle relaxants (eg, meprobamate , Equanil, Robaxin )
- Verapamil hydrochloride (Calan, Isoptin, Verelan)
- Chloroquine phosphate (Aralen)
- Hydroxychloroquine sulfate (Plaquenil)
- Carbamazepine (Tegretol)
- Phenytoin (Dilantin)
To reduce your chance of getting nocturnal leg cramps, take the following steps:
- Stretch three times a day and just before going to bed.
- Face a wall and put your hands on the wall and keep them there. Step backward. Keep your knees locked. Keep your heels on the floor until you feel a strong pull in your calves. Hold that position for 10 seconds. Repeat two or three times.
- Exercise feet and legs regularly.
- Drink plenty of liquids.
- Eat plenty of potassium-rich foods. This includes bananas, tomatoes, potatoes, broccoli, cantaloupe, oranges, and grapefruit.
- Wear comfortable, supportive shoes.
- Sleep with toes up, not pointed downward.
